Consent for Treatment, Assignment of Benefits, and Statement of Financial Policy All Graves-Gilbert Clinic patients will be required to sign our Financial Policy Statement when they come in for an appointment. It is available on our website for your review. |
AT THE CLINIC:If you are new to the clinic, please stop at the main desk in the first floor atrium area.
WEEKEND AND EMERGENCY INFORMATION:There is always a physician on call 24 hours a day. If a medical emergency should occur after hours, on weekends or holidays, call 270-781-5111. Our answering service will contact the on-call physician, who will return your call as soon as possible.We all work as a team, so be assured that in an emergency, the physician on call can effectively manage your problem.
CANCELLATIONS:If you are unable to keep your appointment, please notify the appropriate office no later than 24 hours prior to the appointment. A new appointment can be scheduled for you. There is no charge for a cancelled appointment. | |
