You can make appointments at Graves-Gilbert Clinic in several ways:

Consent for Treatment, Assignment of Benefits,
and Statement of Financial Policy

All Graves-Gilbert Clinic patients will be required to sign our Financial Policy Statement when they come in for an appointment. It is available here for your review.

Your local physician can refer you to Graves-Gilbert Clinic and make the appointment for you.

AT THE CLINIC: If you are new to the clinic, please stop at the main desk in the first floor atrium area.

WEEKEND EMERGENCY INFORMATION:

There is always a physician on call 24 hours a day. If a medical emergency should occur after hours, on weekends or holidays, call 270-781-5111. Our answering service will contact the on-call physician, who will return your call as soon as possible.

We all work as a team, so be assured that in an emergency, the physician on call can effectively manage your problem.

CANCELATIONS:

If you are unable to keep your appointment, please notify the appropriate office no later than 24 hours prior to the appointment. A new appointment can be scheduled for you. There is no charge for a cancelled appointment.
